Qualifications and Responsibilities
- Observation, assessment, and documentation of patient symptoms and progress
- Perform initial home care visits and re-evaluate patient needs and progress when needed
- Demonstrate efficient teamwork with the staff
- Demonstrate competence in documenting Oasis
- Supervise HHA field staff as indicated
- Follow Medicare regulations
- Notify the physician about any changes in the patient’s condition
- Coordinate other disciplines as applicable to assist the patient
- Complete paperwork timely and efficiently
- Educate patients and caregivers about the disease, medications, care plans, and treatment strategies as outlined in the policy and procedure manual
- Deliver high-quality care to patients at home, adhering to established policies and procedures
- Provide wound care and assessment. Provide IV line care and maintenance
